This program is intended to identify to nurses the importance of care plans and how to draft plans that reflect the personal wishes of the patient with still addressing medical conditions, psychosocial problems and potential risks for harm 1. Describe the role of nurses in driving patient care through person-centered care plans. 2. Identify today's challenges for effectively using person-centered care plans. 3. Cite the four remedies for improving the context of nursing care plans. 4. Differentiate between person-centered versus patient specific care plan goals and interventions.
