Course overview
This program for providers and nurses will provide instruction to better understand what EHR quality metrics are already measuring and to identify chronic hypertension patterns in your patient base. It will review possible implications of other diagnoses on the management of hypertension in your patients, especially for obesity and diabetes. Effective use of the EHR can assist providers to identify and select patients, and to design practical interventions using your EHR and other methods to track progress within a chronic disease management program.
