Interdisciplinary Collaborative Practice

Course overview

This module discusses how the hospice and palliative interdisciplinary group interfaces with each other and with other providers in various healthcare settings. It discusses how the hospice plan of care is created and when it is necessary to make changes to the plan of care. On completion of this module, the learner will be able to: Differentiate between the home, hospital and nursing home setting for the dying patient. Describe the process for coordinating patient care with other healthcare providers. List ways that hospice can collaborate with providers including referral sources and facilities. List the advantages of the interdisciplinary tea model, members of the team and goals for advance care planning.
