Discusses errors in healthcare, including related challenges, different factors/forces at play, how healthcare teams can work collaboratively to set realistic goals, and how to engage patients in order to minimize errors. Preventable medical errors are the third leading cause of death in the United States, and cost our country tens of billions of dollars annually. The role of errors within healthcare systems can be complex, with some errors being non-consequential, and others leading to serious injury and/or death. While human error is inevitable and cannot be eliminated, processes can be implemented to help mitigate the frequency, visibility, and consequences when errors occur. This program discusses errors within healthcare systems, including strategies that healthcare systems can utilize to minimize error and adverse events.
