Discusses how to manage the aftermath of errors in healthcare, including the effects that adverse medical events may have on all involved parties, key points related to self-reporting of adverse events, and tips for how organizations can help to build effective root cause analysis (RCA) processes for when an error does occur. Preventable medical errors are the third leading cause of death in the United States, and cost our country tens of billions of dollars annually. The role of error within healthcare systems can be complex, with some errors being non-consequential, and others leading to serious injury and/or death. While human error is inevitable and cannot be eliminated, processes can be implemented to help mitigate the frequency, visibility, and consequences when errors occur. This program discusses the widespread effects that error may have on practitioners as well as organizations, reporting adverse events, and building effective RCA processes for when error does occur.
