Identify the necessary steps and considerations to plan, initiate, and advance PN to goal in the home 2. Describe how to monitor laboratory values in the long-term home PN patient and apply knowledge gained to successfully monitor lab values for maintaining acid/base balance, essential fatty acid deficiency, and micronutrients 3. Demonstrate knowledge on nutrition recommendations to optimize oral intake and nutritional status in patients with intestinal failure NOTE: An annual subscription for Purdue's nutrition support pharmacy re-certification program provides access to both the continuing education component of the program and the "Professional Resources" for practitioners. Subscriptions to the continuing educational offerings, which have been accredited through ACPE and BPS, can be purchased for an annual fee of $195 for pharmacists.
