Hospice in Long Term Care: Hospice Philosophy of Care (REL-HOS-0-HLTCHPC)

Course overview

There are many things to consider when exploring the hospice philosophy of care in the long-term care setting. This webinar will provide information on the differences between hospice and palliative care, the history of hospice, the hospice model of care and how this model of care blends with the long term model of care to help guide an individualized plan of care to meet the physical, spiritual, psychosocial, and emotional needs of the patient and family. Information will also be included on the dying process as well as grief and bereavement and how care is provided to ensure a holistic plan of care. Additionally, as healthcare has advanced, determining prognosis has become more difficult so we will also be discussing situations in which a hospice referral would be appropriate.
