At the end of this update, the reader should be able to: Understand the approach to the laboratory investigation of unprovoked venous thrombosis. Know the major components of the fibrinolytic pathway. Recite the activators and inhibitors of the fibrinolytic pathway. Understand plasminogen activator inhibitor-1 (PAI-1): sites of production, role in fibrinolysis and thrombosis, mechanism of action and common associations. Recall the genetics of the PAI-1 4G/5G polymorphism and its role in fibrinolysis and thrombosis.
