HCAHPS Series: Discharge and Transitions of Care

Course overview

Improved preparation for discharge, care coordination, and transition of care can result in reduced adverse events and readmissions, greater adherence to the plan of care, and better patient experience, leading to increased Hospital Consumer Assessment of Healthcare Providers and Systems ratings and reimbursement. The patient's experience and HCAHPS results are influenced by experiences outside of the hospital. This course educates healthcare providers about patient discharge, transition of care, and care coordination as they pertain to HCAHPS.
