Employ active listening to identify gaps and proactively prepare for continuing care in the home environment. 3. Describe steps needed to implement education to patients 4. Prepare clinicians to recognize barriers to understanding, look for areas that need additional reinforcement. 5. Describe techniques to overcome patient learning challenges. NOTE: An annual subscription for Purdue's nutrition support pharmacy re-certification program provides access to both the continuing education component of the program and the "Professional Resources" for practitioners. Subscriptions to the continuing educational offerings, which have been accredited through ACPE and BPS, can be purchased for an annual fee of $195 for pharmacists.
