This educational monograph addresses the identification, reporting, monitoring and reduction of medical error. The 1999 Institute of Medicine report To Err is Human: Building a Safer Health System brought prominence to the issue of medical error in the US. Since that time multiple activities at various levels of private and regulatory arenas have targeted identification and resolution of medical error. In 2016, researchers from Johns Hopkins University published findings from their study indicating that medical error can now be ranked as the third leading cause of death in the U.S. Health professionals and health systems both need ongoing education in error reduction techniques in order to create and maintain an improved culture of safety. Some regulatory agencies require specific medical error education for licensure or re-licensure of health professionals.
