This course reviews professional perceptions of healthcare transformation models that use Risk Stratification (RS) to manage care resources. Risk stratification is a process that identifies or groups patients to receive care resources. Health care consists of scarce or limited resources, and RS is an approach aimed at creating maximum benefit from the use of those resources. Risk stratification is a process for improving patient outcomes while reducing the overall costs of treatment and usage of health care resources. The method can use human review, computer-based programs, or a combination of both to determine which patients have the most compelling risks for adverse outcomes. RS is a process that places priorities on the use of health care resources to reduce adverse outcomes. Adverse outcomes include preventable diseases and costly complications that may be reduced or avoided completely by early detection, intervention, and treatment. Care managers can assign proactive measures and adequate resources to treat high-risk patients and avoid adverse developments that cause increased levels of disease and mortality among the patient population.
