The interdisciplinary team is important in hospice care. The physician or physician extenders, such as nurse practitioners and physician assistants, are members of the hospice interdisciplinary team. This course discusses the role of the entire interdisciplinary team with an emphasis on hospice physicians. It also discusses the importance of physician documentation for hospice certification and recertification, as well as how clear, concise documentation of hospice eligibility can lead to improved care. Without this information, hospice professionals may fail to meet the regulatory requirements and the hospice may be at risk for loss of certification.
