Course overview
As a healthcare professional, you know the plan of care (POC) is the written guide for patient care ordered by a physician or allowed practitioner. It is important that the POC be complete, accurate, and signed by the attending practitioner. It is also critical that it includes specific components to be in compliance with CMS requirements. In this course, you will get definitions, descriptions of CMS forms, and aids to help you write effective POCs. The goal of this course is to provide knowledge to home health staff related to the CMS-485 Plan of Care requirements.
