Transitions between care sites include transitions within the hospital, from the hospital to home, or from the hospital to another health care organization at the time of discharge. This course presents the concept of transitions of care for infants and children and the implications for pediatric nurses. It explains how transitions of care apply to pediatric post-acute care and describes best practices for care transitions between acute and post-acute care for infants and children.
