Proper discharge planning is necessary to ensure that resident discharges are successful for both the resident and the facility. The entire interdisciplinary team should be involved in discharge planning. Discharge planning should begin upon admission to the facility. This course discusses the discharge planning process, including resident and family education. It also discusses the regulations surrounding discharge planning. Without adequate discharge planning, individuals may not have the necessary medications and supplies needed to maintain their health and safety. The goal of this course is to educate nursing and social work professionals in post-acute care settings on discharge planning.
