Effective Fall Risk Management for Medically Complex Patients

Course overview
Older adults with multiple co-morbidities, especially those with dementia and/or frailty, are at a much higher risk for experiencing a fall or multiple falls. These patients pose a unique and significant challenge to the interdisciplinary team (IDT) in skilled nursing and home health settings, and require a different set of fall risk management processes and priorities. Through a series of applied and practical case studies of complex patients, this course presents evidence-based and best practices for an IDT to manage fall risk in patients with multiple medical fall risk factors. The process of root-cause analysis will be reviewed, and interventions that include all members of the care team to improve outcomes will be discussed.
