Course overview

Medication-related problems are common among patients, especially those who have multiple disease states and complex medication histories. The management of patient medications can be a challenge for all health care practitioners in both the hospital and home care setting. This course will review the steps of the medication reconciliation as discussed in Part One of the series, titled Home Care Medication Management: Performing a Medication Reconciliation. It will include a detailed discussion of the barriers to an effective medication reconciliation and other areas in which the process may fail. This is followed up by how to overcome these hurdles and strategies to improve the process. A relatable case review is provided. This course is applicable to nurses, therapists, social workers, and any other health care professional interacting with patients in home health and/or hospice settings.
