This course is part of our Special Topics series, Complex Clinical Cases. A 59yo female c/o right ankle pain, swelling & limited weight-bearing, post tri-malleolar fracture repair w/ ORIF, with subsequent 2nd MTP stress fracture and concurrent vasomotor instability. Injury occurred 9 weeks earlier. While visiting relatives, she was attempting to descend stairs while carrying bedsheets. Her foot got tangled in the sheets, she tripped & fell, breaking her distal tibia, including tibial posterior rim, & fibula. Course Goals
Upon completion of this course, the Therapist will be able to:
1. Analyze, Understand, and Apply management techniques and interventions to successfully rehabilitate a patient with a Tri-Malleolar fracture and subsequent complications.
2. Analyze and Identify the presence of risk factors for developing an acute MTP fracture.
3. Recognize and Identify the presence of RSD in different stages and management of each.
4. Understand the impact and effect of complications identified in this case (MTP Fracture & RSD) on patient management and clinical outcomes.
Course Objectives
1. Understand the Epidemiology of Tri-Malleolar Fracture and potential complications.
2. Review Critical Anatomy and Biomechanics of the Ankle Joint—normal and post injury anatomy.
3. Dissect and Analyze the case scenario History for clues about Stress Fractures, Mechanotoxicity, and RSD including identification of biomechanics and the mechanisms of injury.
4. Understand the interrelationships of the stress fracture and development of RSD.
5. Interpret the specific Physical Exam findings of the case including special orthopedic tests as well as Static vs Dynamic Load Tolerance testing.
6. Recognize the clinical presentation of RSD, including the different Stages.
7. Understand the Physiology and Pathophysiology of RSD and how it affects treatment.
8. Learn management and monitoring strategies for RSD - in the Chronic Stage.
9. Understand Treatment Triage in cases with multiple tissue-type injuries.
10. Review and Analyze the Treatment Protocol used in this case scenario in terms of: Tissue-types injured and pathophysiology of the tissue injury (including bone, vasomotor system, neuromotor system); principles of tissue-specific rehabilitation in terms of load, forces applied, and progression of the treatment course; therapy used in the case and rationale for each treatment intervention.
11. Learn how to progress a Rehabilitation Program for a Tri-Malleolar fracture case complicated by MTP fracture and RSD from initial evaluation to discharge and HEP based on objective clinical findings.
