Developing an Individualized Patient-Centered Care Plan

Course overview
Every home health clinician should understand certain key elements when developing appropriate interdisciplinary individualized plans of care and managing that care. These elements should include information about the patients health as well as a focus on how to identify patients needs address problems, and evaluate patients responses and progress. The goal of this course is to provide licensed professionals in home health care with guidance for developing interdisciplinary individualized plans of care.
