Medicaid is the U.S.s public health insurance program for over 72.5 million Americans, including individuals with disabilities (Medicaid.gov, n.d.a).This is especially important for individuals with chronic physical conditions to ensure the critical health services they need remain affordable and accessible. This course provides an overview of the management of Medicaid beneficiaries with chronic physical conditions, assists caregivers and/or coordinators in overseeing the chronic physical conditions of Medicaid beneficiaries, and broadens their understanding of their role in the discharge planning process.
