Care Teams: Diabetes

Course overview

Diabetes is widely prevalent, with over 1.4 million people diagnosed in the U.S. yearly (Centers for Disease Control and Prevention [CDC], 2022a). The goal of diabetes treatment is to prevent the development of comorbidities by offering ongoing education and clinical follow up. To achieve this, multiple subspecialties need to be involved in the delivery of the treatment plan. A team-based approach will not only optimize patient outcomes, but healthcare resources too. This course will review the importance of care teams, clinical checkpoints, and the role each healthcare provider must play when caring for patients with diabetes. The goal of this course is to educate the healthcare team in all settings on the benefits of care teams for the management of diabetes.
