Course overview
Diabetes is a common condition. If not controlled, diabetes will lead to serious complications that can decrease quality of life and increase cost to both the patient and the healthcare system. By creating a self-care plan with their patients, healthcare providers empower patients to take a bigger role in their disease management. When patients become empowered, resources (both time and economic) can be thoughtfully allocated to other patients in need. A self-care plan requires input from both the healthcare provider and the patient, and considers factors such as finances, living situation, diet, or other social determinants of health. In this course, the actions needed for a well-rounded self-care plan will be explored using story-based learning. The goal of this course is to educate the healthcare team in the acute care setting on diabetes management utilizing self-care plans.
