CE PROVIDER INITIAL APPLICATION REQUIREMENTS (CSWMFT Board-approved provider):

Note : This application must be received by the Board at least 60 days prior to the first scheduled program offering.

• Your initial provider application will require the submission of one (1) course. Note: Provider Approval authorizes the provider to offer CE to all CSWMFT Board licensees.
• Submit the resume from only one CSWMFT licensee(s). This licensee will review and approve all CE offered by the Provider. This licensee is approving CE for all professions licensed by the CSWMFT Board. Please attach no more than 3 pages of a current resume for the licensee who will approve/review programs.
• Attach a list of at least 2 or 3 additional program topics or titles. No additional details regarding the planned programs or topics are required. This list will provide the Board with a broader view of the continuing education to be offered.

CE PROVIDER COURSE APPLICATION REQUIREMENTS (Ohio CSWMFT Board Approved Provider):

• The following documents must be submitted ELECTRONICALLY with the course submitted as part of an initial provider application for review:
• Attach Presenter(s) resume, no more than 3 pages. If more than one presenter, please combine into one PDF; agenda with content and timeframes, do not attach a power point; evaluation form; CEU certificate with program information; post-test (with answers) if not live.
• If an asynchronous course, please attach a 5-question post-test for every 1 hour of content with answers.  Include how the number of awarded hours were calculated.
• Certificate of completion which indicates: the name of the provider, the program title and date(s), place for licensees name (to be added by your agency), number of clock hours awarded to the participant, specify if the hours meet the ethics or supervision requirement if applicable, statement of approval by the counselor, social worker and marriage and family therapist board and space for approval number assigned by the Board, signature of an official representative of the provider.
• A program agenda and brochure, which delineates the content and timeframes of the program including start/end and breaks for each session
• Sample program evaluation form

You will be asked to submit a CE Provider profile and primary contact information. You will also be required to attach proof of programs being approved by NBCC.

COURSE APPLICATION REQUIREMENTS (Providers with programs approved by NBCC):

Courses do not route for approval. Once registered, courses will be auto-accepted after attestation of course being approved by the NBCC.

You will be asked to submit a CE Provider profile and primary contact information. You will also be required to attach proof of having programs approved by the Ohio Chapter of NASW .

COURSE APPLICATION REQUIREMENTS (Provider with programs approved by the Ohio Chapter of NASW ):

Please be prepared to attach proof of course being approved by the Ohio Chapter of NASW

You will be asked to submit a CE Provider profile and primary contact information. You will also be required to attach proof of programs being approved by NASW/Washington.

CE PROVIDER REGISTRATION REQUIREMENTS (Provider with programs approved by ASWB) : When registering as a Rule/Statute approved CE provider, CE Broker will lead you through a simplified registration process. You will be asked to submit a CE Provider profile and primary contact information. You will also be required to attach proof of programs being approved by ASWB.

COURSE APPLICATION REQUIREMENTS (Providers with programs approved by ASWB):

Courses do not route for approval. Once registered, courses will be auto accepted after attestation of course being approved by the ASWB

CE PROVIDER REGISTRATION REQUIREMENTS (Educational provider approved by a state board of marriage and family therapy)

If you are directly approved by another state board of marriage and family therapy you can use that approval to become approved by the OH CSWMFT board to offer CE to the professions that are regulated by the board you currently hold approval for.

*Please Note: You must be approved directly by the state board.

Please be prepared to attach proof of approval documentation from another state board of marriage and family therapy.
