CE PROVIDER INITIAL APPLICATION REQUIREMENTS (FPTA APPROVED COURSE PROVIDER):

  • When applying as an FPTA approved course provider, CE Broker will lead you through a simplified registration process. You will be asked to submit a CE Provider profile and primary contact information. You will not be required to submit attachments or remit payment for this initial registration.


  • Once registered please log into your account to submit your course(s) for approval. Be prepared to submit the following documentation with your course application for review:
  • Course brochure and/or schedule
  • CV(s) or resume(s) of speaker(s) clearly indicating credentials in area of course content and license numbers. Please note that bios included in brochures or advertising are not sufficient.
  • Copy of certificate of completion
  • Program outline that must fully describe the time devoted to each topic area, including program objectives. The objectives of the program should clearly indicate the relevance to physical therapy practice.
  • A current reference list used for course in AMA or APA format is required. Upon review of course, additional materials may be requested.
  • It is the intent of the Continuing Education Department of the FPTA to ensure that material being submitted for course approval is current and evidence based to the extent that there is research to support the content. If research is not available, a written summary of why evidence based research is not available and a summary of relevance and theory with reference citations is required.


  • Course Providers must include the following statement on the course materials:  Accreditation of this course does not necessarily imply the FPTA supports the views of the presenter or the sponsors.

    COURSE APPLICATION REQUIREMENTS(BOARD APPROVED PROVIDER):

    3. Instructor's Advanced training/Certification specific to course content
    4. Copy of certificate of completion
    5. Program outline that must fully describe the time devoted to each topic area, including program objectives. The objectives of the program should clearly indicate the relevance to physical therapy practice.
    6. A current reference list used for course in AMA or APA format is required. Upon review of course, additional materials may be requested.
    7.     Course fees are as follows :

    Up to 3 hours = $125.00
    4-7 hours = $250.00
    8-15 hours = $350.00
    16 and up = $500.00

    One CEH = 50 minutes
    .5 CEH = 25 minutes
    CEU conversion : 1 CEU = 10 CEH hours
  • CE PROVIDER REGISTRATION REQUIREMENTS(RULE/STATUTE APPROVED PROVIDER):
  • Please attach proof of accreditation if registering as an Educational PT or PTA program accredited by the Commission on Accreditation of Physical Therapy Education (CAPTE) or proof of approval if registering as a Provider of courses sponsored or approved by the American Physical Therapy Association or any of its components (other than the Florida Physical Therapy Association).
